Registration Information and Procedures
Please consider registering online. This is a fast and simple way to complete your meeting registration. Payment must be made at the time on-line registration is completed. Unpaid on-line registrations will automatically be canceled.
- If not registering on-line, complete the registration form included with the registration brochure. A separate form must be used for each primary registrant. Copies of the form may be made if additional copies are needed. All information must be completed in full, using blue or black ink. Please print clearly.
- ARBO OE Tracker Number – please include your ARBO OE tracker number on your registration form or verify it is accurate if registering on-line. This number is designed to track all optometrists’ continuing education activity in one central database. Any licensed optometrist may go to www.arbo.org to obtain his/her OE Tracker number. OE Tracker numbers are not required to register for Optometry’s Meeting™.
- Calculate the base registration fee due for each category, as appropriate, on the registration form.
- Calculate the subtotal due for functions selected each day; calculate total due for all days.
- Add base registration fee(s) from above.
- Calculate the grand total due by adding all function fees, plus base registration fee(s). This is the total amount due for your registration..
- Payment in full must accompany the registration form and must equal the amount shown in Grand Total. Registrations received without payment will not be processed. On-line registrations must make payment by MasterCard, VISA, or American Express. Mailed registration forms may make payment by check (U.S. dollars only) made payable to Optometry’s Meeting™ or by MasterCard, VISA, or American Express. The AOA has the authority to adjust the amount charged to credit cards if the total of all functions requested and/or base registration fee(s) is incorrect.
- Continuing education fees have changed this year. Registrations postmarked on or before April 1, 2008, are eligible for the early-bird discounted rate for continuing education courses, $30 per hour. Registrations postmarked from April 2, 2008 thru May 31, 2008 will be processed at the regular rate for continuing education courses, $35 per hour. Registrations postmarked after May 31, 2008 will be processed on-site at the rate of $40 per hour. You will be charged the rate as determined by your postmark.
- On-line registrations submitted on or before 11:59pm CST, April 1, 2008, qualify for the early-bird discounted rate for all continuing education courses, $30 per hour. On-lineregistrations submitted from April 2, 2008 thru May 31, 2008, will be processed at the regular rate, $35 per hour.
- The deadline for advanced registration is May 31, 2008. We are unable to process registrations received after May 31.
- We encourage all registrants to submit their registration as early as possible in order to receive the early-bird discounted hourly education rate, availability of courses, and to avoid long lines on-site in Seattle. Registrations are processed in the order received.
- Confirmation of registration will be sent to each registrant prior to the mailing of the registration materials. Please allow 2-3 weeks for receipt of confirmation after registering. Please review your confirmation quickly and thoroughly and correct any errors prior to the mailing of your registration materials. - Attention, AOA Attendees: Registration materials — name badge, social function, education tickets, and other important information — will be mailed under separate cover prior to Optometry’s Meeting™ to the address provided on the registration form. Please remember to bring all materials with you to the meeting.
- Attention, AOSA Attendees: Your registration materials must be picked up on-site at the AOSA Registration Desk at the Washington State Convention and Trade Center.
- There is a $25 fee to reprint lost or forgotten Registration materials.
